Thalassemia blood donors: regular help in Quetta

Children with thalassaemia major need blood about once a month, for life. Why the same few donors wear out, and how to become a regular donor.

Short answer

People with beta thalassaemia major may need a blood transfusion about once a month (NHS). In Balochistan the disease is common, so the families of these patients look for donors again and again. A regular donor who gives a few times a year, when it is their turn, is one of the most useful things a healthy person in Quetta can be.

Why thalassaemia patients need blood again and again

Thalassaemia is an inherited blood disorder. In its severe form, beta thalassaemia major, the body cannot make enough healthy red cells, so the patient lives on donated blood. People with beta thalassaemia major may need a transfusion about once a month (NHS).

Regular transfusions also build up iron in the body, which can damage organs, so patients take treatment to remove the extra iron (NHS). For the family, it means planning blood, medicine and hospital visits every few weeks, for years.

Thalassaemia in Balochistan and Pakistan

At a 2016 meeting reported by Dawn, the secretary general of the Thalassaemia Federation of Pakistan put the prevalence of the disease in Balochistan at 8 percent and said there were 60,000 thalassaemia patients in the country. The coordinator of the Punjab thalassaemia prevention programme put the national carrier rate at 5.4 percent, with around 6,000 children born with the disorder every year (Dawn, 2016). These are the most recent figures we could verify; there is no national registry.

Which donors can give to a thalassaemia patient

The same red cell rules apply as for any patient: the patient’s own blood group first, then groups whose red cells the patient can safely receive. Our blood group compatibility chart lists them. The hospital tests and cross-matches every bag, and the patient’s doctor decides what is given.

Why the same donors burn out

A family that needs blood every month usually leans on the same few relatives and friends. They are called every time, they give as often as they are allowed, and after a year or two they are tired, unwell or simply out of reach. Then the family starts again with public posts and lists.

Donors need rest between donations. NHS Blood and Transplant asks men to wait at least 12 weeks and women at least 16 weeks between whole blood donations (NHS Blood and Transplant). A family needing a bag a month therefore needs several donors in turn, not one.

Can you give?

Register once. We ask only when your blood can help.

You are contacted only when a patient who can receive your blood group needs blood, never within six months of your last donation, and your number is never published.

How to become a regular thalassaemia donor

  1. Know your blood group. If you do not, the blood bank checks it when you give.
  2. Check that you can give. Most blood banks in Pakistan accept healthy donors from 18 up to about 60, weighing at least 45 to 50 kg (Husaini Blood Bank). See the full rules.
  3. Register once on Khidmat, so families can reach you when a patient who can receive your blood needs it.
  4. Mark each donation. Khidmat then rests you for six months, so you are never asked too soon.

How Khidmat spreads the asking

On the Blood Bank page, a family asking for blood sees donors offered in turn: the patient’s own group first, and within each group the donor asked longest ago. Nobody is asked twice while others wait, and anyone who gave in the last six months is not shown. The load moves round many donors instead of falling on the same few.

Questions people ask

How often do thalassaemia patients need blood?

People with beta thalassaemia major may need a transfusion about once a month, according to the NHS. Their doctor sets the schedule.

How often can I donate for a thalassaemia patient?

Blood services set a minimum gap: NHS Blood and Transplant uses 12 weeks for men and 16 for women. Khidmat waits six months after you mark a donation before showing you again.

Can I donate if I am a thalassaemia carrier?

Ask the blood bank: they check your haemoglobin and decide on the day.

Written by the Khidmat team. Medical facts are checked against the sources listed above. This guide is general information, not medical advice: the doctor and the hospital blood bank decide who can give and receive.
